Many years ago, I had such a bad case of Tinnitus that I couldn't sleep 
at night.

I was so distraught that I'd wake up crying from frustration.

I had access to excellent Western Physicians.  My father was an esteemed 
physician in the Bay Area
and I had what is called 'professional courtesy'  which meant that I 
could go see top specialists who new
my father, personally or by reputation.

The long and short of it was that I was diagnosed with sever Tinnitus 
and that there was really very little
to do for it , except for just ceasing, completely to be around loud 
music.   This was not great news
as I was a successful professional musician as you are now.

Then I asked my acupuncturist about it.

Anyway,  she turned me onto a therapeutic regimen that completely 
changed all of this around.

Chinese medicine is very controversial in the Cartesian oriented Western 
Science field, 
so I don't even feel like getting flamed on this thread for suggesting 
what I will suggest to you,
but if you will contact me offlline,  I'll be happy to share with you 
what I learned from her
and what solved my Tinnitus situation.

There are also Chinese herbal and acupuncture approaches to helping with 
TMJ as well and there
definitely seems to be a correlation between the two.

It might be well worth your while to do a video chat consultation with 
my acupuncturist.
She is phenomenal and the best in this area which is an area known for 
Acupuncture study.

It would not take a lot of money to consult with her and I can't more 
highly recommend that you hear
what she has to day.

Additionally,  I have discovered that there are many side effects to the 
overuse of caffeine.

I am a coffee addict, myself but now limit myself to 2 strong cups a day 
at the very most
because I started having all kinds of symptoms from drinking to much 
coffee,  including, notably,
stiffness and pre-arthritis conditions in my fingers.

Pulling back off the caffeine had a definite effect.  I even when cold 
turkey for a few months
which was efficacious.

I know your pain around this issue.  It totally tore me up for a while 
15 years ago.
